The SS-4 builder prepares an EIN application PDF; it does not issue an EIN or submit an application to the IRS. Establish the applicant’s legal identity, responsible party, federal classification, reason for applying and payroll expectations before entering the fields. EINs themselves are available free from the IRS.

Reviewed October 29, 2026 · The tool uses the December 2025 continuous-use Form SS-4 revision, including its original two-page packet. The example is fictional and makes no application on anyone’s behalf.

The SS-4 application PDF builder organizes information for a separately submitted application. It is useful to distinguish the document being prepared, the entity being identified and the government-issued number eventually requested.

What does the builder provide, and what does the IRS provide?

The service fills the supported SS-4 fields and offers a complete PDF preview before the document order. The IRS decides the application and assigns the EIN. Paying a private document-preparation charge is not an IRS application fee, a reservation of a number or evidence that an EIN has been assigned.

The IRS direct EIN application page provides the free government route and its eligibility conditions. The online service has requirements concerning domestic formation, principal business location, the responsible party or authorized representative, and an appropriate taxpayer identifier. Applicants outside that route should review the official alternative procedures.

Form the corporation or LLC through the appropriate state process before applying when that applies to the applicant. Preparing SS-4 does not form an LLC, register a trade name or make a corporation election. Those actions have their own requirements.

Check whether a new EIN is actually needed. A different business location or a new employee does not automatically require another number for an entity that already has one. Resolve an existing-number problem before treating a new application as the solution.

How should the name and address fields be prepared?

Use the applicant’s legal name in Line 1 and the business trade name, when different, in Line 2. Gather the formation or other legal records first. The person completing the worksheet is not necessarily the entity requesting the EIN.

The mailing address identifies where correspondence should go. A different physical address has separate fields; do not use a post-office box as the physical street address. The principal-business county and state are another location entry, not a substitute for the complete mailing address.

For an executor, trustee or care-of arrangement, make sure the appropriate person belongs in the designated field. Foreign addresses need complete country and local address details. Verify the printed result rather than assuming that a short screen label contains every required address component.

The builder includes an “already assigned EIN” field. Leave it blank for a genuinely new application unless the IRS has already assigned the number for that application. The separate previous-EIN question concerns an earlier number received by the entity; it is not permission to invent an identifier.

Store actual taxpayer identifiers privately. A planning article or shared example should not contain a real owner’s SSN, ITIN or business EIN.

Who is the responsible party?

The responsible party is the person with the relevant ultimate ownership or effective control over the entity and its assets. It is generally a natural person; government entities have a specified exception. The IRS responsible-party guidance also explains why a formation nominee with limited authority should not be listed in that role.

A registered agent, bookkeeper or person typing the application is not automatically the responsible party. Review actual authority rather than selecting whoever can provide a convenient tax identifier.

Line 7b normally calls for the responsible individual’s SSN or ITIN. The official instructions permit the specified foreign/N/A entry when the responsible party lacks and is ineligible to obtain those identifiers; a government entity may use an EIN under its exception. “I would rather leave it out” is not the same condition.

Keep the responsible-party choice separate from a third-party designee. The latter may receive and discuss application information under the particular authorization. It does not replace the identity of the person who actually controls the applicant.

Review the legal applicant, responsible individual and federal tax classification before preparing Form SS-4; submit the application separately to the IRS.
The applicant, controlling person and federal classification answer different application questions.

Does selecting LLC determine the federal tax classification?

No. The LLC questions identify legal organization, membership count and domestic formation. The federal classification is a separate entry. A corporation, partnership and disregarded entity can have different treatment even when state law uses an LLC structure.

The SS-4 classification instructions explain default classifications and special entries. For example, a domestic single-member LLC treated as disregarded generally uses the specified “Other” description for the relevant application purpose, while an elected corporate treatment needs its appropriate classification information.

Do not check “sole proprietor” simply because an LLC has one owner without reviewing the required SS-4 procedure. A foreign-owned U.S. disregarded entity applying for Form 5472 reporting has a particular description; foreign ownership should not be hidden by a generic selection.

The builder reveals fields for the selected type, such as a corporation’s expected return form, a trust’s grantor information or an explanation for another entity type. Prepare those facts before choosing the category. The document does not make an underlying tax election merely by displaying a form number.

What might a fictional new partnership application worksheet contain?

Assume a newly formed domestic LLC has two individual members and is classified as a partnership under the applicable default rules, with no corporate election. It has completed its state formation, has never received an EIN and needs one for its new operating business. These are assumed facts, not a recommendation for every two-owner arrangement.

The application’s business-start date, accounting closing month, principal activity and description of products or services must also match the actual facts. A December closing month should come from the permitted tax-year determination, not a preference for a familiar month.

Write a specific description of the work performed rather than repeating only “business” in every descriptive box. The activity category and the detailed description should agree. If the selected application reason requires an explanation, supply the actual reason-specific information.

How do employee counts connect to the annual Form 944 option?

Line 13 asks for the highest expected number of employees in each category during the next 12 months. These are headcounts, not dollars, owners’ percentages or last year’s W-2 total. Enter zeros in the applicable boxes when no employees are expected.

For the fictional LLC, assume the two ordinary employees are expected to receive $24,000 each in a full calendar year, all subject to ordinary Social Security and Medicare tax and below the individual Social Security cap. Total wages would be $48,000.

Combined employer/employee Social Security and Medicare tax alone would be $48,000 × 15.3% = $7,344, before adding any federal income tax withholding. Under those assumptions, the expected liability is already above $1,000, so the application’s small-liability Form 944 choice is not appropriate.

This is a planning check, not an actual payroll return. Changes in hiring, wage taxation or expected annual wages require a fresh review. The first-wage-date entry must also reflect the required facts, rather than automatically repeating the formation date.

Where the SS-4 annual-filing election is available and properly made, follow its specific instructions and the resulting IRS requirement. Existing employers should not infer a change in filing frequency from this new-application checkbox. Our Form 944 annual-return guide covers checking the assigned requirement, and our Form 941 guide covers quarterly preparation.

In the fictional payroll forecast, two employees at24000 dollars each give48000 dollars wages and7344 dollars combined Social Security and Medicare tax before income tax withholding, exceeding the1000 dollar Form944 planning limit.
The forecast already exceeds $1,000 before income-tax withholding; it does not support the small-liability choice.

What should happen after the PDF review?

Check the current official SS-4 original against the completed application. Inspect the legal name, every address, responsible party, classification, reason, dates, headcounts, previous-number answer and applicant contact details.

The tool can leave signing for printing or apply the applicant’s own drawing. Complete the certification using the requirements for the actual submission route and the proper authorized signer. A paid document order does not prove signing authority.

Review whether a third-party designee is authorized before entering their details. Keep the scope of that authorization separate from broader tax representation and from the responsible-party role.

Choose the applicable IRS submission method and current destination from the official instructions. Avoid sending duplicate applications through several channels to speed processing. The service checkout and the emailed PDF link are not IRS submission confirmations.

Once an EIN is actually assigned, retain the IRS confirmation with the entity records. Do not present an unsigned application, order receipt or preview as proof that a number has been issued.

SS-4 PDF builder questions

Must I pay the IRS for an EIN?

No. The IRS issues EINs free. A private PDF-preparation price is separate from the government application.

Does the December 2025 revision mean the application is only for tax year 2025?

No. It is the form’s revision, not an annual payroll reporting period. Verify the current edition when applying.

Can a formation nominee be the responsible party for convenience?

Do not substitute a limited-authority nominee for the person who meets the IRS control definition.

Does selecting LLC automatically elect corporate taxation?

No. State organization and federal classification are distinct; any applicable election has separate requirements.

Will the completed PDF contain a newly assigned EIN?

The builder does not issue a number. IRS assignment and confirmation occur through the separate application process.

The Check Stubs Online Editorial Team checked the December 2025 builder, official application and responsible-party guidance. The original forecast verifies 2 × $24,000 = $48,000 and $48,000 × 15.3% = $7,344 under ordinary fully taxable wage assumptions.
